Welcome to Centennial Boys Lacrosse Club! We are excited to have you join our growing program. Whether you are new to the sport or returning for another season, this is where your journey begins. Our registration process ensures every player is insured, rostered, and ready for competition.
Player Registration Process
Step 1 Join USA Lacrosse
All players must have an active USA Lacrosse membership for insurance and eligibility. Join or Renew at USA Lacrosse.
Step 2 Complete CEN10 LAX Registration
After confirming your USA Lacrosse number, complete the Centennial registration form. You will enter player details, emergency contact info, and pay the club registration fee.

Step 3: Submit the completed form and proceed to the payment section to finalize your registration.
Participation Requirements
Completed Registration Form
Ensure all fields are filled out accurately and completely.
Proof of Age
Submit a copy of the player’s birth certificate or state ID.
Medical Clearance
Provide a signed physical examination form from a licensed physician.
Code of Conduct Agreement
Review and sign the player and parent code of conduct agreement.
Payment Confirmation
Complete the payment process to confirm your registration.
Emergency Contact Information
List at least two emergency contacts with current phone numbers.
Uniform Sizing Form
Fill out the form to ensure proper uniform fit for the season.
Waiver and Release Form
Sign the waiver to acknowledge understanding of program risks and responsibilities.

What Age groups Are Eligible?
Visit our registration page and follow the step-by-step instructions to complete the online form and submit payment.
Do I need to buy my own gear?
Yes, players are responsible for their own equipment. However, the club offers rental gear and financial assistance if needed.
How much does it cost to play?
Fees vary by season and age level. We work hard to keep costs low and provide payment plans when possible.
What if I am new to lacrosse?
Perfect. We welcome beginners and provide coaching focused on skill development, confidence, and teamwork.
Are there any additional fees?
The registration fee covers most expenses, but there may be additional costs for optional tournaments or special events.
Refunds are available up to two weeks before the season starts, minus a processing fee.
What are the payment options available?
We accept credit/debit cards and PayPal. Payment plans are also available upon request.
What equipment does my child need?
Players need a lacrosse stick, helmet, gloves, shoulder pads, and cleats. We provide jerseys and shorts.
